According to the sources, the easiest pairs to match in Kidney Paired Donation (KPD) programs are those with high biological and immune system compatibility. The people who are easiest to match usually fall into these groups:

  • Recipients with Common Blood Types: People with common blood types are much more likely to find a match quickly. O-type recipients have the hardest time because they can only get kidneys from O-type donors, while people with A or B blood types have more options.
  • Low-Sensitized Recipients: People with low sensitization, meaning they have few or no HLA antibodies against donors, are the easiest to match. Those without HLA sensitization have about a 1 in 4 chance of finding a match in a typical pool.
  • Unsensitized ABO-Incompatible Pairs: Pairs who are only incompatible because of blood type, and not because of tissue-type antibodies, do very well in registries. For these non-O recipients, the chance of finding a match in a 2-way or 3-way exchange is about 60%.
  • Recipients with Multiple Potential Donors: Someone who brings more than one willing donor into the program, even if only one ends up donating, has a much better chance of finding a match quickly.
