What Are the Important Principles of Vascular Access for Hemodialysis?

Managing vascular access (VA) for hemodialysis is a coordinated part of the patient’s long-term care, not just a technical fix.

  1. The Initial Principle: Prevent Central Catheters for a Healthy Access Lifeline

The main principle is avoiding central catheters, temporary or permanent, because they often cause injury and stenosis, and sometimes complete obstruction of the central veins. This prevents AV fistula and graft construction and leads to a significant loss of healthy vascular access lifespan projections.

Prevention of Catheters

Three main items are important for the follow-up of AV fistulas and grafts:

  1. Timely construction of an AV fistula, ideally almost a year before the possibility of initiating hemodialysis.
  2. Yearly follow-up of the access by a transplant surgeon performing Doppler ultrasound and documenting flow and complications such as aneurysms, with timely intervention before thrombosis occurs.
  3. Immediate reconstruction of the AV fistula or graft on the day of thrombosis to prevent central line insertion.

  • Creation Plan:
    This plan covers what type of access (AVF, AVG, or CVC) will be used and when it will be placed, based on vessel mapping.
  • Contingency (Complications) Plan:
    This plan prepares for problems such as stenosis, thrombosis, or infection and lists steps to address them if they occur.
  • Access Succession Plan:
    This plan prepares for the next access if the current one fails, so the patient’s veins are not used up too soon.

  • Multidisciplinary Surveillance:
    Individualized care, especially after a transplant, depends on a team of nephrologists, cardiologists, and VA specialists to balance the risk of heart problems with the need for future dialysis.
  • Timely Intervention:
    Regular monitoring helps identify urgent issues, such as thinning skin over an aneurysm or dead tissue, which require immediate referral to prevent dangerous bleeding.
