What Is the HERO Graft?
The Hemodialysis Reliable Outflow (HERO) graft is a special type of vascular access device for patients who have limited veins or have used up their peripheral vein options. It is called a hybrid graft-catheter because it combines a regular dialysis graft with a catheter.
Key Features and Components
- Composition:
The device is made of a polytetrafluoroethylene (PTFE) graft connected to a silicone catheter for venous outflow. - Placement:
The entire device is placed under the skin. - Mechanism:
The graft connects to an artery, and the catheter portion passes through the central veins—even if they are narrowed—and ends in the right atrium of the heart.
Clinical Indications
The HERO graft is mainly used as a last-resort vascular access for patients who cannot have a traditional arteriovenous fistula or graft because of:
- Poor-quality peripheral veins.
- Central vein narrowing or blockage, which usually prevents the use of a standard graft or fistula in that arm or leg.
- Used-up access sites, where the HERO graft is a more reliable option than long-term dialysis catheters.
Benefits Compared to Standard Catheters
Although it uses a catheter-like outflow, the HERO graft has several advantages over traditional central venous catheters (CVCs):
